Change in Peak Systolic Velocity Estimation in Internal Carotid Artery Following Contralateral Near Occlusive Carotid Endarterectomy

چکیده

Carotid artery stenosis (CAS) is one of the main causes stroke, as internal carotid (ICA) a major source cerebral blood supply. A common surgical intervention for CAS endarterectomy (CEA), which carries high risk stroke and mortality. CEA indicated 50% to 99% in symptomatic patients 60% asymptomatic patients. duplex ultrasound frequently used estimate peak systolic velocity (PSV), end diastolic (EDV), ICA/common (CCA) ratio determine extent stenosis. This does not account other factors that can artificially change PSV EDV, such contralateral near occlusion with >90% Although >70% has been previously studied, limited research exists on impact ICA occlusions. We hypothesize degree an ipsilateral patent vessel be more accurately estimated by examining pre- vs post-CEA occlusion. retrospective study was conducted 239 patients, at least 18 years age, who underwent standard, routine single institution. Patients qualified had bilateral occlusive disease side CEA. Near determined either following criteria: >400, EDV >140, or ICA/CCA >6, computed tomography angiography showing interpreted physician. Preoperative velocities were recorded compared postoperatively. Interestingly, presence >50% stenosis, PSV, unoperated increased significantly (P < .001). In greater than 70% occlusion, detecting ≥70% pre-CEA, ≥250 cm/s 87.2% sensitivity. predicting >300 96.0% The present deviates from current theoretical model shows ratios elevated pre-CEA provides novel insight into customized criteria estimation avoid unnecessary high-risk surgery.

Sustained bilateral hemodynamic benefit of contralateral carotid endarterectomy in patients with symptomatic internal carotid artery occlusion.

BACKGROUND AND PURPOSE We sought to investigate whether in patients with a symptomatic internal carotid artery (ICA) occlusion, endarterectomy of a severe stenosis of the contralateral carotid artery can establish long-term cerebral hemodynamic improvement. METHODS Nineteen patients were studied on average 1 month before and 6 months after contralateral carotid endarterectomy (CEA). Volume fl...

Carotid angioplasty and stenting in a patient with high grade stenosis of Internal Carotid Artery associated with both vertebral arteries and contralateral carotid occlusion

Severe internal carotid artery (ICA) stenosis is a common cause of cerebrovascular accident (CVA) in middle-aged patients. Contralateral carotid occlusion (CCO) in patients with severe ICA stenosis is associated with high risk of CVA. Carotid endarterectomy (CEA) is associated with more complications in patients with CCO than those without CCO. In this study, we present the case of a 61-year-ol...

What happens to the external carotid artery following carotid endarterectomy?

AIMS The effect of carotid endarterectomy on the patency of the external carotid artery is unknown. We conducted a retrospective study to evaluate the long-term changes in the external carotid artery following carotid endarterectomy. METHODS Data was prospectively recorded for all patients who had carotid endarterectomy between 1997 and 2006 in our vascular surgical unit. These patients had f...

Doppler ultrasound assessment of the internal carotid artery following carotid endarterectomy.

Doppler-shifted ultrasound with spectral analysis was used to assess the internal carotid arteries of 48 patients who had undergone carotid endarterectomy (58 carotid endarterectomies). Three patients died in the immediate post-operative period, therefore 45 patients, who represented 54 endarterectomies, were assessed for up to six years following surgery (mean--thirty-four months). Ultrasound ...
